WESTLAKE WOMEN’S CLUB MEMBERSHIP AGREEMENT, LIABILITY DISCLAIMER, AND PHOTO RELEASE
By signing below, I hereby apply to become a member of the Westlake Women’s Club, a nonprofit organization. I understand and agree to the following terms:
1. Membership Purpose
I understand that the Westlake Women’s Club is a volunteer-based nonprofit dedicated to community service, social enrichment, and charitable activities. As a member, I agree to support the Club’s mission and participate in its activities to the best of my ability. I also agree to abide by the rules set forth in the Club’s bylaws as specified in the Club’s yearbook.
2. Voluntary Participation
I acknowledge that my participation in all club meetings, events, programs, and volunteer opportunities is completely voluntary. I understand that some activities may involve physical activity, travel, or interaction with the public.
3. Assumption of Risk and Release of Liability
I understand and agree that participation in Club events may involve certain risks, including but not limited to physical injury, illness, property damage, or other unforeseen events. By joining the Club and participating in its activities, I voluntarily assume all such risks.
To the fullest extent permitted by law, I release and hold harmless the Westlake Women’s Club, its officers, directors, members, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, injury, or illness that may occur during or as a result of my participation in any Club-related activity.
4. Code of Conduct
I agree to conduct myself in a respectful and cooperative manner during all Club events and communications. I understand that the Club reserves the right to suspend or terminate my membership for behavior that is disruptive, harmful, or inconsistent with the mission and values of the organization.
5. Permission to Use Personal Image (Photo Release)
I hereby grant the Westlake Women’s Club permission to photograph, film, or otherwise record my image and likeness during Club activities. I authorize the Club to use, reproduce, and publish such images in print and digital formats, including but not limited to newsletters, social media, websites, marketing materials, and news media, for promotional or informational purposes.
I understand that:
I will not receive compensation for the use of my image.
My name may be used in connection with such images unless I notify the Club in writing to withhold it.
This release remains in effect unless I revoke it in writing.
